Observation

110-300-0165(2)(d)- there were zip loc plastic bags in children cubbies with clothes inside and there was a plastic bag in cubbie with diapers inside, area was accessible to children. 110-300-0165(3)(a)- outside licensed area on the face there were bolts facing towards license space. 110-300-0165(3)(f)- two cabinets in children area were not anchored to the wall 110-300-0165(5)(a)- outlets in the kitchen area were not tamper resistant.
